Valid Passport:

You must send the actual passport. The passport must signed and have at least six (6) months validity remaining before expiration and has at least one complete blank visa page for the visa stamp.

Visa Application Form:

Download, completely fill-out and sign Brazil visa application form. Two forms per applicant.

Please note that Brazilian Consulate in Houston will not accept visa application forms distributed from other consulates. The signature on the visa application form must be original, not faxed or photocopied.

Photograph:

You must provide two 2"x2" passport type photos. Homemade Digital Photographs are not acceptable.

Proof of residence in the consular jurisdiction:

You must send a notarized copy of your driver license or your state ID, and you must have resided in one of the following states: Arkansas, Colorado, Kansas, Louisiana, New Mexico, Oklahoma, Texas for the past 12 months prior to your visa application.

Non Criminal Record:

Provide a recent non-criminal record issued by your local Police Department.

Other Documents:

• Copy of scholarship documentation or proof of financial capability to pay for the stay in Brazil.
• Documented proof of enrollment, legalized by a notary public in Brazil, indicating duration of studies at a Brazilian educational institution.
