Valid Passport:

You must send the actual passport. The passport must signed and have at least six (6) months validity remaining before expiration and has at least two complete blank visa pages for the visa stamp.

Visa Application Form:

Each applicant must complete an eVisa Application for Brazil on SCEDV web site . On the home page of SCEDV web site, click "VISA REQUEST" under "VISA" Section on the left menu. An application number will be assigned to you once you have completed entering the information. You are required to submit a signed receipt. The signature on the receipt must be original, not faxed or photocopied. Also, the signature should match the one in your passport.

Minors under age 18 must submit a notarized copy of birth certificate and a notarized Travel Authorization Form for Minors . Application receipt for minors must be notarized and signed by both parents.

Photograph:

You must provide one 2"x2" passport type photo. Homemade Digital Photographs are not acceptable.

Business Letter & :

Applicant's original business letter from the employer or sponsoring company - on the letterhead stationary and signed by a senior manager which contains the following information. The signature on the letter must be original.

• Nature of business of the U.S. company.
• Applicant title and/or functions with the U.S. company and his/her precise nature of business and activities to be conducted in Brazil
• Responsibility of the U.S. company for the applicant's stay in Brazil and his/her return to the country of origin.
• Name(s) and address(es) of Brazilian companies with which the applicant is going to deal/negotiate.
• Name(s) of person(s) to be contacted in Brazil, phone number and his/her/their functions with the Brazilian company.
• Duration of stay of their immediate trip to Brazil.

Letter of Invitation from company in Brazil:

A business letter of responsibility from your partner in Brazil is required:

•The letter must be addressed to the Consulate of Brazil.
•The letter must explain the nature or purpose of business and the duration of the proposed business trip.
•The letter must state the name as well as the addresses of individuals, departments and organizations in Brazil you intend to visit.

Copy of the Driver License:

You must provide one clear copy of your driver license.

Travel Documentation:

Send a copy of your travel itinerary or airline ticket.
