Valid Passport:

You must send the actual passport. The passport must have at least six (6) months validity remaining before expiration and has at least two complete blank visa page for the visa stamp.

Additional Requirements for Minor:

All minor applicants must submit a copy of both parents passport.

Photograph:

You must provide two 2"x2" passport type photo, No glasses to be worn in any photo.

Proof of Address:

Applicants must submit one clear copy of their utility bill or driver license. For Minor, copy of parent's driver license is acceptable.

Copy of the birth certificate:

Applicants must submit one clear copy of their birth certificate. For Non-US born applicants that do not have their birth certificate, provide a copy of a document issued by Government authorities. This document could be an old (not current) passport or a school leaving certificate etc., provided it shows their personal particulars like name, date of birth, parents names, and nationality.

Persons of Indian Origin:

Persons of Indian Origin or a spouse or dependent child of a person of Indian origin can not apply tourist visa. The applicant must apply entry visa.

Additional Requirements for Non-U.S. Citizens:

You must send a copy of both sides of your U.S. Green Card or your U.S. visa.
