If your passport was issued more than one year ago, you can not make name change on your currenct passport, and you should use the passport renewal instruction.

Step 1 Gather and Prepare U.S. Passport Application Documents

Completed DS-5504 form

Please click on the link at the bottom of this section to complete, and create your passport application (DS-5504) online. After you fill out the form online, you will see at the top of the page a button that says "Create Form", please click the "Create Form" button, and your application form (DS-5504) will appear completed on your screen.

1. Please print one copy of the completed application form (DS-5504).
2. Do not print the form on double sided pages.
3. You must sign the completed application form (DS-5504).

If you don't have any travel plan, or your departure date is not within 14 days, please make sure do not fill in the Travel Plans section online, and leave it blank.

Your current valid U.S. Passport

You must submit an original marriage certificate, divorce decree or court order showing the change of name. The name change document must bear the official seal of the issuing authority. Uncertified copies or notarized documents can not be accepted. All documents will be returned to you with your passport.

Taken within the past 6 months, showing current appearance. Must be official passport-type only and approximately 2" x 2" with a clear background. The photo must be in color. Do not wear a hat or any item that covers the face.Face forward only, no side shots or profiles will be accepted. Homemade Digital Photographs are not acceptable.

Note: You may take your passport photos at Kinko's, Walgreens or CVS.

You must provide one of the following items:

• A copy of your air plane ticket or
• A copy of your itinerary from the travel agent or airline or
• If you do not have a ticket or itinerary please click here

Download and complete two copies of Authorization Letter. This letter will authorize Visa Express to submit and obtain the new passport on your behalf.

1. U.S. Government Fees:

Make one check or money order payable to "U.S. Department of State" for $60 (this is the government processing fee).

a. Write applicant's full name and date of birth on the check or money order.
b. All personal checks must show a printed address.
c. Do not cross out and rewrite any information on the check.
d. Temporay or starter check is not acceptable.

2. Visa Express Service Fee & Return Shipping Fee:

Our Rush Passport Service fee is $139. It can be paid with credit card, company check or money order made out to "Visa Express ".

Our return shipping fee is $23 for Fedex Overnight service. $38 for Fedex Saturday delivery, $65 for Fedex 8 AM delivery.

You may combine our service fee and return shipping fee in one check or money order or pay by credit card. .

Step 2 Ship Documents to Visa Express

Please place the following documents in a FedEx or UPS Express envelope.

• DSP-5504
• Your valid passport
• Two passport photos
• Name change document
• Proof of departure
• Letter of authorization (two copies)
• Order Form
• The $60 check or money order to "U.S. Dept. of State"
• Payment to "Visa Express"

Ship your package via Fedex Priority Overnight only.
          Do not use U.S. Postal Service Express Mail.
